Financial Assistance

• Pennsylvania Housing Finance Agency: Offers no-cost personal financial education for renters and homeowners, as well as loans for homeowners.
• Property Tax/Rent Rebate Program: The rebate program benefits eligible Pennsylvanians age 65 and older; widows and widowers age 50 and older; and people with disabilities age 18 and older.

Utility Assistance and Weatherization

• Pennsylvania Low-income Home Energy Assistance Program (LIHEAP): Offers financial help to low-income households to pay their heating bills

• UPMC Homelessness Continuum Program: Serves families and individuals in Allegheny County who are homeless and living with a mental illness and/or substance abuse disorder

Nursing Home Transition Program

The UPMC Community HealthChoices Nursing Home Transition Program assists UPMC CHC participants who want to move from a nursing facility to a home of their choice in the community.
